What couples counselling is, and what it is not
Couples counselling is talking therapy for two people together. The subject is what happens between you, not which of you is right.
The approaches share a relational frame 1. The counsellor keeps both people in view. The counsellor builds the working alliance with the pair, not with one partner. That is the practical difference from individual therapy, and the reason both of you attend.
It is not arbitration, and nobody hands down a ruling at the end. Couples counselling is for couples married, engaged, living apart, or still deciding whether to stay. Relationship distress is an ordinary reason adults seek therapy 2.
How does a session with two people work?
Most couples work happens with both partners in the room. That is the standard arrangement, and the counsellor sets clear rules in advance about any separate contact with either partner 1. Ask about those rules early. They decide what happens to anything you say on your own.
Sessions are commonly weekly and about an hour long. Many approaches expect the work to run for some months rather than years. Both people speak. Expect the counsellor to ask about the history of the relationship, not only about the argument that brought you.
Approaches differ, and the main models of couples therapy produce broadly similar results for relationship distress 2.
Who is qualified to do this work?
India has more than one register, and which one applies depends on the practitioner's category.
A clinical psychologist must hold registration with the Rehabilitation Council of India, which issues a registration number and maintains the central register 3. Psychologists and counsellors in other categories fall under the National Commission for Allied and Healthcare Professions 4. The NCAHP Act 2021 provides for a register under that Commission, which we could not reach when we checked. A psychiatrist is a registered medical practitioner, and the National Medical Commission publishes the Indian Medical Register 5.
Counselling itself is not covered by a single national title. Ask which body a practitioner is registered with, what that registration covers, and ask for the registration number. Where that body publishes a register you can reach, check the name on it.
Titles you will see include Clinical Psychologist, Rehabilitation Counsellor and Rehabilitation Psychologist.
What a couples session costs
We do not set fees. Practitioners set their own, and a couples session is often longer than an individual one, which moves the fee.

Read it before you enquire. Then ask each practitioner what a couples session costs, how long it runs, and whether the fee differs from an individual session.

Is couples counselling the right first step?
Sometimes the first appointment should not be a joint one.
Moderate to severe aggression between partners is one reason 1. Active alcohol or other substance misuse is another. So are a continuing affair and psychotic symptoms.
A careful practitioner asks about these before agreeing to see you together. They may suggest individual or specialist help first.
If you are afraid of your partner, safety comes before counselling. The services listed below exist for exactly that. Seeing someone on your own first is a common way in, not a decision about the relationship.
Couples counselling across India
Marriage counselling and couples therapy name the same work. The practitioners listed here cluster in Delhi, Bangalore and Mumbai, and most record Hindi alongside English.

What does marriage counselling cost in India?
Four things move a fee. The practitioner's registration category, their experience, the session length, and whether you meet in a room or online.
Where a practitioner states a fee on their listing, that figure is their own. Ask before the first session. Ask too whether the fee runs per session or per person in the room.
Does marriage counselling work?
Couple therapy is an effective treatment for relationship distress, and the main models reach broadly similar results2.
Behavioural, cognitive-behavioural, emotionally focused and integrative behavioural couple therapy meet the criteria researchers use for a well-established treatment9.
Those findings describe groups of couples in studies. They are not a forecast about any one relationship, and nobody listed here can promise you an outcome.
What the research does not settle is which approach suits you. Ask a practitioner how they work with two people, and what they do when the two of you want different things.
What happens in a marriage counselling session?
The practitioner works with what happens between you, not with one person's account of it.
Practice differs on what comes first. Some approaches open with a formal assessment that combines joint and individual meetings. Others use no set assessment stage1.
A first meeting normally covers three things. Why you are coming now, how each of you sees the problem, and what each of you wants changed.
Ask before you start whether the practitioner sees each partner alone. Ask how you will know the work is done.
Will health insurance pay for marriage counselling in India?
The Mental Healthcare Act, 2017 requires every insurer to make provision for medical insurance for the treatment of mental illness. It is to be on the same basis as cover for physical illness. India's insurance regulator put that to all insurers by circular10.
Marriage counselling is not itself treatment of a mental illness. That direction does not settle whether a session with a couple is claimable on your policy.
Ask your insurer whether it covers outpatient consultations with your practitioner's category, and ask the practitioner what they can invoice.
When should we go for marriage counselling?
There is no threshold you have to cross first. Relationship distress is one of the commonest reasons adults seek psychotherapy at all2.
Distress between partners travels. It is associated with mood, anxiety and substance-use difficulties in the partners themselves, and with difficulties for children in the household1.
Couples come with a repeating argument, with an affair, with a decision about whether to stay together, and with none of those. You do not have to agree on the problem before you make contact.
If the problem is safety rather than the relationship
Couples counselling is not the route when someone is being hurt or frightened at home. India has services for that, and they are separate from anything on this page.
The Women Helpline of the Ministry of Women and Child Development runs at all hours 11. It refers callers on to the police, hospitals, legal aid and a One Stop Centre. A One Stop Centre is a single building where medical help, legal aid, police assistance, counselling and short-term shelter sit together 12. For a child, the national child helpline operates through district child protection units 13.
The legal route is civil. Under the Protection of Women from Domestic Violence Act, a magistrate grants the orders 14.
The district Protection Officer's job is to help a woman make the complaint and record the incident. The same officer prepares a safety plan and connects her to legal aid, medical care and shelter. The National Legal Services Authority provides free legal aid 15.
