What couples counselling is, and what it is not
Couples counselling is talking therapy for two people seen together, with the relationship itself as the work.
The approaches share certain features 1. A relational frame. An active style. Work on the patterns that keep a difficulty going, rather than on who is at fault.
It is not family mediation. Mediation settles arrangements for children or money, and a court usually expects a mediation information and assessment meeting before an application about child arrangements 2. Counselling settles nothing and reports to nobody.
In England the NHS offers couple therapy where one partner has depression, free and by self- referral 3. Relationship distress is an ordinary reason adults seek therapy 4.
How does a session with two people work?
Both partners usually attend. Sessions run about an hour, most often weekly, and most approaches expect months of work rather than a single conversation 1.
Each approach sets its own rules for contact with one partner alone. Ask in your first email how the practitioner handles a private call or message from one of you.
No one model beats the rest for relationship distress 4. Ask how a practitioner works, and whether it fits the way the two of you talk.
Who is qualified to do this work?
The law protects psychologist titles. Only a registrant of the Health and Care Professions Council may use the titles clinical, counselling, educational and health psychologist, and the Council regulates practitioners rather than practices 5.
The same register holds the arts therapies titles, among them art psychotherapist and dramatherapist. Counsellor and psychotherapist are not protected titles. The Professional Standards Authority accredits their registers, UKCP and BACP, which are not statutory 6.
Psychiatrists are doctors, and the General Medical Council holds the medical register 7. Each nation of the United Kingdom regulates social work separately 8.
Couples work is not a separate registration, so check the register for the profession, then ask about experience of seeing two people together.

The usual reasons are moderate to severe aggression between partners, active alcohol or other substance misuse, a continuing affair, and psychotic symptoms 1. Individual or specialist treatment comes first.
If one of you is afraid of the other, that is a safety question first. Going alone is a sensible beginning, not a sign you have given up on the relationship.

You can ask the police whether a partner has a record of abuse. That is the right to ask under the Domestic Violence Disclosure Scheme, and someone may ask on behalf of a friend or relative.
For protection through a court in England or Wales, you can apply for a non-molestation order or an occupation order online, by email or by post 11. Where a child is at risk, contact the children's social care team at the local council; Wales, Scotland and Northern Ireland have their own route 12. A mediation meeting is not required before a court application where there has been domestic abuse 2.
