What therapy costs in California in 2026

American therapists set their own fees, and California's spread is the country's widest. What is published is worth more than any guessed average: the federal benchmark below, the fees California practices publish themselves, roughly $150 to $300 a session at the practices listing rates, and the public routes through Medi-Cal and the county departments that serve people regardless of what they can pay.

These are guiding estimates. Therapists set their own fees.

The one published reference point

Medicare, the federal payer, allows about $167 for a 60-minute psychotherapy session in 2026 and about $114 for 45 minutes, national amounts adjusted by locality. That is not what private practice charges, but it is a fair benchmark to hold any quote against. If you have Medicare: Part B covers outpatient mental health, paying 80% of the allowed amount once the deductible is met; your 20% share of a 60-minute session is about $33. Since 1 January 2024, licensed marriage and family therapists and licensed mental health counselors can enrol in Medicare and bill independently, at 75% of the clinical psychologist rate, roughly $125 a session with your share around $25.

The public routes

Medi-Cal, California's Medicaid programme, covers mental health care for its members at no or low cost, and the county layer is the practical door: Los Angeles County's Department of Mental Health, the largest in the country, describes its round-the-clock Help Line as "the primary entry point for behavioral health resources", covering crisis assessment, referrals and connection into services. Every California county runs a mental health plan on the same model; searching your county's name with "mental health department" finds its access line.

What practices charge

Among California practices listing their prices: Here Counseling, in Pasadena and Los Angeles, publishes a practice range of $150 to $345 an hour, noting that "many clients work with therapists charging between $180 and $225", and in San Francisco psychologist Dr. Bear Korngold publishes $300 for a 50-minute individual session. These are the practices' own published rates, read on 28 July 2026; they are examples, not a survey, and the working range they suggest is $150 to $300 for an individual session, with senior specialists in the big coastal cities at and above the top of it. Asking about sliding scales costs nothing.

Low-cost routes that publish their prices

Open Path Collective, a nonprofit network, states its terms on its own site: therapy at "$40-70 per session" after a one-time $65 membership fee, online or in person. University training clinics across California run supervised sessions at reduced fees on the same logic: a clinician in training, supervised by licensed staff, at a fraction of market rates.
