The number that matters most in Malaysia is not a private fee. In a government hospital, third-class psychiatric care costs nothing at all. The federal gazette prints the word Percuma: free.

What the law actually says

Charges at Malaysian government hospitals are set by statute, under the Fees Act 1951 and the Fees (Medical) Order 1982 as amended by the Fees (Medical) (Amendment) Order 2017. The gazette sets the psychiatric ward at RM 15 a day first class, RM 10 second class and free at third class, and psychiatric treatment per admission at RM 1,125 first class, RM 300 second class and free at third class: the ordinary subsidised tier for Malaysians in the public system. Penang's government hospitals work to this same national schedule.

The outpatient registration fee sits in a different schedule we could not read at source; it is widely reported as RM 1 at a general clinic and RM 5 at a specialist clinic, with citizens aged 60 and over exempt, and we mark those figures as indicative rather than verified.

Who is qualified, and who regulates the fee

Counsellors are registered under the Counsellors Act 1998 with Lembaga Kaunselor Malaysia, whose statutory functions include regulating the fees registered counsellors charge. Psychiatrists are medical doctors; clinical psychologists are regulated separately. Which of the three you book determines both what they can do and what you will pay.

If cost is the obstacle

The public system is the answer far more often than people assume: a referral through a government clinic to a specialist psychiatric clinic costs a nominal registration fee, and third-class admission costs nothing.
