Therapy in Milan can cost anywhere from free public services to over €250 per session in private practice. This guide breaks down typical prices, cost differences across the city, and practical ways to find affordable support.

What therapy costs in Milan in 2026

Italy funds therapy through two doors. The bonus psicologo pays your psychotherapist up to €50 per session, to a personal cap of €1,500, €1,000 or €500 depending on your ISEE. The public system (SSN) runs mental health care through your GP and the local centro di salute mentale, at regional ticket rates with broad exemption categories. Private fees outside both have no published schedule.

These are guiding estimates. Practitioners set their own fees.

Therapy should be personal. Therapists listed on TherapyRoute are qualified, independent, and free to answer to you – no scripts, algorithms, or company policies.

Find Your Therapist

The bonus psicologo, from INPS's own page

  • The contribution is paid to the professional, up to €50 per psychotherapy session. The therapist must be a registered psychotherapist on the Albo who has joined the scheme.
  • Your cap depends on ISEE: below €15,000, up to €1,500; between €15,000 and €30,000, up to €1,000; between €30,000 and €50,000, up to €500. Above €50,000 you are not eligible.
  • Applications go through INPS online (SPID, CIE or CNS) in an annual window, and funding is ranked by ISEE until the money runs out; the 2025 window ran 15 September to 14 November.
  • If accepted you receive a unique code, valid 270 days. Since 2025 there is a sharper rule: have your first session within 60 days of acceptance or the benefit lapses and passes to the next person in the ranking.

The practical reading: the bonus is real money but rationed, and the calendar matters as much as the ISEE. When a window opens, apply early; at equal ISEE, earlier applications rank higher.

The public route

The SSN's mental health door in Milan is your GP, who can refer you to the local centro di salute mentale for psychiatric care and, where available, psychological treatment. Costs run on the regional ticket system, with exemptions by income, age and condition; your region's health service lists the current amounts. What the public route charges lightly in money it can charge in waiting, and capacity varies widely between services.

The private market

Private psychotherapy in Milan is priced practice by practice, and no Italian body publishes a fee schedule; the Ordine degli Psicologi sets professional rules, not prices. Pages quoting a standard Milan session rate are quoting themselves. The certain figures are the state's: €50 a session through the bonus, and the ticket at the CSM. For anything else, the reliable number is the practice's own quote, and the bonus question is worth asking any psychotherapist directly: se aderisce al bonus psicologo.
